Your journey to

with 5 simple steps
Step 1:
Your food diary. To get a clear sense of your eating patterns, you will be asked to keep a diary of all the
foods you eat for 5 days.
Step 2:
Your health evaluation. Dr. Strencosky and her staff will gather your health history and perform a complete
exam.
Step 3:
Your analysis. Dr. Strencosky will thoroughly analyze your health history and food diary, as well as your
weight loss and overall health goals.
Step 4:
Your customized plan. Dr. Strencosky will develop a nutrition and exercise plan that is customized to your
individual needs and goals.
